You may request to see or obtain copies of your health record.

  • Requests must be in writing.
  • We will respond within 30 days.
  • Reasonable copying fees may apply.
Right to Request a Correction

If you believe your medical record is incorrect or incomplete, you may request an amendment.
We may deny the request under certain circumstances, but we will explain why in writing.

Right to Request Confidential Communications

You may request that we contact you at:

  • A different address
  • A different phone number
  • A specific email

We will reasonably accommodate such requests.

Right to Request Restrictions

You may request restrictions on how we use or share your PHI.
Although we will consider your request, we are not required to agree—except for certain situations where you pay out-of-pocket in full.

Right to an Accounting of Disclosures

You may request a list of certain disclosures that we made without your authorization, for up to six years.

Right to a Paper or Electronic Copy

You may request a paper or electronic copy of this Notice at any time.

Right to File a Complaint

We may update this Notice at any time.
Revisions apply to all PHI we maintain, past or present.
The effective date at the top of this Notice indicates the most recent version.
We will post updated versions on our website and in our office.
